Fissure means crack in the wall. Anal Fissures are small cut or crack in the lining of the anal opening and this causes pain or bleeding while defecating. These fissures are normally caused by constipation and passing of large and hard stool but may also be caused by anal intercourse, insertion of foreign object in the rectum, chronic diarrhea, straining while defecating, child birth through normal delivery.
The symptoms of anal fissure are pain or blood while passing the stools, pain or itching in the anal opening, skin tag near the anus opening. On getting any such symptoms you should immediately consult a good ano-rectal specialist who would be able to diagnose the disease on physical examination. The diagnosis of anal fissures normally do not require any investigation except proctoscopy which may cost you Rs. 2500.
Do not delay in consulting a specialist doctor at the first sign of anal fissure as the problem may get aggravated and the time for non-surgical treatment may get wasted.
Fresh Fissures may be treated non-surgically by eating fibre rich diet and using laxatives to pass the stool without straining. Non Surgical treatments also include some oral medications, local ointment. Sometimes the surgeon may use Nitroglycerin, lidocaine hydrochloride (Xylocaine) or Special injection.
Chronic Anal Fissures are mostly treated by surgery. In this surgery a small portion of the anal sphincter muscle is cut to reduce the spasm and allowing it to heal. Some of the surgeons who have the luxury of Diode Laser at their centre are now doing Laser Surgery for Anal Fissures. The advantages of Laser Surgery are that it can be done in OPD settiings and the patient need not get admitted. Since no anaesthesia is given so no preparation is required for the Laser Surgery. There are no cuts or wounds and there is no blood loss so the patient goes home in few hours and can resume work from the next day.
